MEDIA RELEASE

  I hereby assign full copyright and ownership of all media taken of me to Head 2 Head Hoops LLC (hereinafter referred to as "the Company"). If I am a minor, I affirm that I have obtained consent from my parent or guardian to make this assignment, whose signature is below.

  I grant the Company, its licensees, or assignees the permission to use the mentioned media either separately or together, wholly or in part, with perpetual, irrevocable, and unrestricted rights for editorial, trade, product advertising, and other fashion/business purposes, in any manner and medium. The Company, its media specialists, licensees, or assignees may have unrestricted use of the media for any purpose, including advertising, with any retouching or alteration without restriction.

  I acknowledge that the media and any reproductions thereof shall be deemed to represent an imaginary person, and I agree that the media specialist or any person authorized by or acting on their behalf may use the mentioned media or reproductions for advertising purposes or to illustrate any wording, with no such wording being attributed to me personally unless my name is used.

  I undertake not to prosecute or institute proceedings, claims, or demands against the Company, media specialist, or their agents regarding any usage of the media. If I am a minor, my parent or guardian also agrees to this undertaking.

  I hereby release all media specialists and the Company from all claims and liability relating to images, videos, or photographs taken of me. If I am a minor, my parent or guardian also releases all media specialists and the Company from such claims and liability.
