LIABILITY WAIVER

  I, the undersigned participant, or if a minor, hereby voluntarily and willingly agree to participate in the H2H Hoops organized by the community and Head 2 Head Hoops LLC (hereinafter referred to as the "Activity").

 

In consideration for being permitted to participate in the Activity, I, or if a minor, my parent or legal guardian on my behalf, agree to release and discharge the Activity, its owners, employees, agents, volunteers, and all other persons or entities associated with it from any and all liability, claims, demands, actions, or causes of action whatsoever arising out of or related to any loss, damage, or injury, including death, that may be sustained by me or to any property belonging to me while participating in the Activity, whether caused by negligence or otherwise.

 

I acknowledge and understand that participating in the Activity involves certain risks, including, but not limited to, the risk of injury or death, and I, or if a minor, my parent or legal guardian on my behalf, voluntarily assume all such risks.

 

I, or if a minor, my parent or legal guardian on my behalf, certify that I am physically fit and have no medical conditions that would prevent my full participation in the Activity. I, or if a minor, my parent or legal guardian on my behalf, agree to abide by all rules and regulations governing the Activity, and I agree to follow all instructions given by the Activity's staff or representatives.

 

I understand that this waiver and release is binding upon my heirs, executors, administrators, and assigns, and it is intended to be as broad and inclusive as permitted by the laws of the state/province/country in which the Activity takes place. If any portion of this waiver and release is held to be invalid, the remaining provisions shall continue to be valid and enforceable.

 

I have carefully read and voluntarily signed this waiver and release of liability, or if a minor, my parent or legal guardian has done so on my behalf, and acknowledge that I understand its contents.
